Introducción: La obsesión por la delgadez y la insatisfacción corporal pueden llevar a las adolescentes a seguir dietas sin supervisión, lo que podría asociarse además a conductas riesgosas de control de peso, tales como ayuno, vómitos, uso de diuréticos y laxantes. El objetivo fue explorar conductas de control de peso en adolescentes dietantes y relacionarlas con insatisfacción corporal (IC) y obsesión por la delgadez (DT). Pacientes y Método: Estudio transversal en 439 adolescentes de colegios públicos de Valparaíso diseñado para indagar conductas de control de peso riesgosas a través de escalas de IC y DT del Cuestionario de Trastornos Alimentarios (EDI-2), comparando dietantes y no dietantes. Resultados: El 43% había realizado dieta sin supervisión médica. Las dietantes presentan valores superiores en DT e IC. El 29,6% de ellas presenta restricción alimentaria de riesgo moderado a alto, tomando como base el criterio de expertos, y se encuentran diferencias en la presencia y severidad de las conductas purgativas de control de peso entre los dos grupos estudiados. Conclusiones: Un tercio de las adolescentes estudiadas hacen dietas sin supervisión profesional, presentan mayor IC y DT y conductas riesgosas de control de peso. Aquellas con sobrepeso y obesidad realizan dietas más restrictivas y conductas de control de peso más riesgosas.


Introduction: Obsession with thinness and body dissatisfaction can lead adolescents to follow unsupervised diets, which could result in risky weight control behaviors such as fasting, vomiting, use of diuretics and laxatives. The aim of the current study is to examine weight control behaviors in dieting adolescents and relate them to body dissatisfaction (BD) and obsession with thinness (OT). Methods: A cross-sectional study was conducted on 439 adolescents from Valparaiso public schools to investigate risky weight control behaviors due to BD and OT scales from the Eating Disorders Inventory-2 (EDI-2), comparing restrained eaters and non-restrained eaters. Results: A total of 43% adolescents had followed a weight loss diet without medical supervision. The dieters had higher BD and OT values. Moderate to severe food restriction, based on expert judgment, was observed in 29.6%, and differences in the presence and severity of purging behaviors were found between the 2 groups. Conclusions: One third of the adolescents studied followed diets without professional supervision and had higher BD and OT values, as well as risky weight control behaviors. Overweight and obese adolescents followed more restrictive diets and developed riskier weight control behaviors.

ABSTRACT

This paper concerns a cross-sectional population-based study conducted with adults living in the city of Pelotas, Rio Grande do Sul State, Brazil. It aims to determine the prevalence of weight-loss practices and use of substances for weight-loss during the 12 months preceding the interview. The prevalence of weight-loss attempts was 26.6%. Although dietary control and regular physical exercise were the most commonly used strategies, the prevalence of the combined use of these methods was only 36% for individuals trying to lose weight. The prevalence of use of substances for weight-loss was 12.8% (48.4% of those who tried to lose weight). The use of dietary control and substances was more common among women, while men practiced physical exercise with greater frequency. Teas were the most frequently used substances for weight-loss. Multivariate analysis identified being female, excess weight and self-perception of excess weight as major associated factors for the use of substances for weight-loss. Finally, we found that, although weight-loss attempts are common, the majority of obese individuals do not make attempts to lose weight and only a minority follows the recommended practices.


Estudo transversal de base populacional conduzido com adultos residentes na cidade de Pelotas, Rio Grande do Sul, Brasil, que objetivou determinar a prevalência de práticas de emagrecimento e uso de substâncias para emagrecer, nos últimos 12 meses antes da entrevista. A prevalência de tentativas de emagrecimento foi de 26,6%. Controle dietético e prática regular de exercícios físicos foram as estratégias mais frequentes, mas apenas 36% daqueles que tentaram emagrecer combinaram-nas. A prevalência do uso de substâncias para emagrecer foi de 12,8% (48,4% daqueles que tentaram emagrecer). Mulheres utilizaram controle dietético e substâncias mais frequentemente do que homens, enquanto estes utilizaram mais exercícios físicos. As substâncias de uso mais frequente foram os chás. A análise ajustada identificou sexo feminino, excesso de peso e percepção do excesso de peso como os maiores fatores associados ao uso de substâncias. Assim, identificamos que tentativas de emagrecimento são frequentes, porém, não são referidas pela maioria dos obesos e apenas uma minoria daqueles que tentam emagrecer segue as práticas recomendadas.

ABSTRACT

Evidence has shown the negative effects of unsupervised diets and those with excessive calorie restriction. The aim of this study was to determine the proportion of adolescents engaging in weight loss dieting and associated factors. This was a cross-sectional study of 4,452 adolescents born in Pelotas, Rio Grande do Sul State, Brazil, in 1993. The outcome was defined as adolescents that reported having practiced some type of weight loss dieting in the previous 12 months. Prevalence of such dieting was 8.6 percent (95 percentCI: 7.7;9.4), and was higher in girls. Elevated maternal body mass index (BMI) was associated with dieting among girls. The adolescent's and parents' view of the adolescent's weight, excess weight, and consumption of diet or light soft drinks were associated with adolescent dieting. There was a positive association between dieting and socioeconomic status. The findings provide important backing for policies aimed at improving adolescents' diet, since they express a major concern over weight and thus a significant percentage of individuals with erroneous and unhealthy behaviors.


Evidências mostram efeitos negativos de dietas sem orientação e de alta restrição calórica. O objetivo do estudo foi determinar o percentual de adolescentes que realizaram regime de emagrecimento e os fatores associados a este comportamento. Estudo transversal incluindo 4.452 nascidos em Pelotas, Rio Grande do Sul, Brasil, no ano de 1993. Os adolescentes que relataram ter feito algum tipo de regime com a finalidade de emagrecimento nos últimos 12 meses foram positivos ao desfecho. A freqüência de realização de regime foi de 8,6 por cento (IC95 por cento: 7,7;9,4), sendo maior entre as meninas. O índice de massa corporal (IMC) materno elevado associou-se à realização de regime entre meninas. O sentimento do adolescente e dos pais em relação ao peso do jovem, o excesso de peso e o consumo de refrigerantes diet/light mostraram-se associados ao desfecho. Encontrou-se associação positiva entre regime e nível econômico. Tais achados constituem-se um importante subsídio para formação de políticas a fim de melhorar a dieta de adolescentes, uma vez que expressa a grande preocupação com o peso, e assim, um porcentual significativo de indivíduos tendo comportamentos equivocados.

ABSTRACT

Objective. The main purpose of this study was to identify the trends of risky eating behaviors among student population in Mexico City in the period 1997-2003 and its relationship with sex age and family head educational level. Methods. Data from the 1997 (N = 9,755), 2000 (N = 3,286) and 2003 (N = 3,062) Drug and Alcohol Prevalence in Student Population of Mexico City Survey's were analyzed. These are representative data of students of junior high, high school and technical schools in Mexico City, with a mean age of 14.5 years (SD - 1.9). Eating behaviors were assessed with a brief questionnaire developed following DSM-FV diagnostic criteria. Risky eating behaviors and the number of risk indicators were compared by sex, age and family head educational level. Results. The results showed an overall increase in the percentage of males (from 1.3% in 1997 to 3.8% in 2003), and females (3.4% in 1997 to 9.6% in 2003) who showed 3 or more risky eating behaviors. The analysis by age groups showed an increase in all but 16-17 year old males, and in all females, being 12-13 year old males and 18-19 year old females the most affected. Regarding the type of risk indicators in both sexes and across all age groups, an increase in preoccupation towards gaining weight and the use of purging and restrictive methods to loose weight was observed. Body weight over estimation decreased in males, while in females showed an increase along with binges in the 2000 survey but showed a decrease after that (2003 survey). The relationship between the family head educational level and risky eating behaviors did not show a regular pattern among males, while a positive relationship between both variables was found among females. Conclusions. A first overall view of the situation of risky eating behaviors among adolescent students in Mexico City and the changes observed in the period studied. An increase of these behaviors was observed along the period of study, being younger males and older females the most affected, as well as women whose family head showed higher educational level.


Objetivo. Identificar las tendencias de las conductas alimentarias de riesgo en estudiantes del Distrito Federal en el periodo 1997-2003 y su relación con el sexo, la edad y el nivel de escolaridad del jefe de familia. Material y métodos. Se analizaron los datos de las encuestas sobre la Prevalencia del Consumo de Drogas y Alcohol en la Población Estudiantil del Distrito Federal 1997 (N = 9,755), 2000 (N = 3,286) y 2003 (N = 3,062). Se trata de muestras representativas de estudiantes de nivel secundaria, preparatoria y técnico en la ciudad de México, con una edad promedio de 14.5 años (DE = 1.9). La información acerca de conductas alimentarias de riesgo se obtuvo mediante un cuestionario breve desarrollado con base en los criterios diagnósticos del DSM-IV. Se hicieron comparaciones por sexo, edad y escolaridad del jefe de familia acerca del tipo, cantidad y frecuencia de conductas alimentarias de riesgo. Resultados. Los resultados mostraron un incremento general en la proporción de estudiantes que reportaron tres o más conductas alimentarias de riesgo (hombres, 1.3% en 1997 a 3.8% en 2003; mujeres 3.4% en 1997 a 9.6% en 2003). Por grupos de edad, se observó incremento en la frecuencia de estas conductas en varones de todas las edades excepto los de 16 y 17 años, y en las mujeres de todas las edades, siendo los más importantes en los varones de 12-13 años y en las mujeres de 18-19 años. En cuanto a los tipos de conducta, se incrementaron la preocupación por engordar y el uso de métodos purgativos y restrictivos para bajar de peso, la sobrestimación del peso corporal disminuyó de forma general en los varones, mientras que en las mujeres se incrementó junto con la práctica de atracones en la medición 2000, pero disminuyó en la medición 2003. La relación con la escolaridad del jefe de familia no mostró un patrón regular en los varones, pero en las mujeres se encontró una relación positiva entre estas variables. Conclusiones. Se muestra un primer panorama acerca de la situación que guardan las conductas alimentarias de riesgo en los adolescentes escolarizados de la ciudad de México y los cambios observados en el periodo estudiado. Se observa un incremento de dichas conductas a lo largo del periodo de estudio, siendo los varones más jóvenes y las mujeres de mayor edad los má-s afectados, así como las mujeres cuyo jefe de familia refirió mayor nivel de escolaridad.

ABSTRACT

El término alimento dietético es muy utilizado a nivel profesional y general, pero no se le menciona en la reglamentación nacional o en el Codex Alimentarius. A nivel general, existe la percepción de que se trata de alimentos formulados especialmente para situaciones como la diabetes, el sobrepeso o la obesidad dado que no contienen sacarosa o porque aportan menos calorías. Si bien al formular alimentos con menos calorías se logra un mayor impacto reemplazando o eliminando las grasas, también es posible hacerlo a través del reemplazo de los carbohidratos, utilizando para ello aditivos conocidos como edulcorantes, en particular los de tipo no nutritivo. Para los más representativos de ellos se ha descrito usos, capacidad o poder endulzante y la Ingesta Diaria Admisible (IDA). Estudios realizados en adolescentes italianos e individuos brasileños conducentes a evaluar la ingesta de edulcorantes no nutritivos no muestran una superación de las respectivas Ingestas Diarias Admisibles. Por otra parte, existen antecedentes que indican que un porcentaje importante de la población prefiere mantener sus hábitos alimentarios y por lo tanto, opta por productos que mantengan el sabor dulce pero sin las calorías. Desde este punto de vista, el concepto de alimento dietético efectivamente representa una contribución importante en la prevención, tratamiento y manejo de la obesidad

En esta presentación se hace referencia al concepto, epidemiología, fisiopatología y tratamiento de la obesidad. Se destaca la importancia del manejo integral de la obesidad a través de la motivación, orientación dierética, actividad física y modificación de conducta. En la orientación dietética se revisa la importancia de la fibra dietaria en el tratamiento. Finalmente se consideran las medidas de prevención de la obesidad.
